The Coalition offers those who register...
•  access to a network of others concerned with end-of-life issues
•  information about programs related to the end of life in Westchester
•  awareness of current topics in the end-of-life field

There is no fee to register but we encourage those who register to make a voluntary (tax-deductible) contribution.

Training as a volunteer for “Everybody Needs a Proxy”

Registration is required.  To register, please complete the Proxy Training Form online, or download and mail the printable form to the Coalition.  If you have questions, please contact us. 

Our affiliate organizations need patient care volunteers and other services. Contact us and tell us your volunteer goals in the end-of-life field. We will help you fulfill them!
